Keeping your information safe ... what you need to know
This page explains why information is collected about you and the ways in which it may be used. The health professionals looking after you keep records about the care and treatment you have received. These records ensure you get the best possible care and typically include information such as your name, address, date of birth, details of your care and results of any tests or investigations.
How your records are used to help you?
Your records are used by our staff to ensure that:
- The professionals involved in your care have accurate information to make sure you get the most appropriate treatment both now and in the future.
If you need to be referred to a specialist or professional then we can provide them with the necessary information to plan your care.
Any concerns you have about your treatment can be properly looked into.
We can look at the health trends of our population and plan future care.
Review the care we provide to ensure it is of a high standard.
Where necessary in these situations your personal details will be removed so that you cannot be identified.

HOW DO I GAIN ACCESS TO MY OWN RECORDS?
The Data Protection Act 1998 allows you to find out what information is held about you on computer and in certain manual records. The 'right of subject access' applies to your health records. If you would like to see them then you should write to the organisation which holds them. You are entitled to receive a copy but note there is likely be a charge. Some access may be limited if it is in your own interest or for other reasons.
